We are able to accept same day emergency
referrals although planned placement are preferred. Information on
each child will be required that includes the LAC documentation,
previous review meeting minutes, observations on general behaviours,
risk assessments etc. It is hoped that the young person will visit the home together with the social worker/family members to view the house and the service we offer and to minimise anxiety and stress. Admissions should be arranged in a planned manner that is beneficial to the individual needs of the child or young person. All placing authorities will be expected to arrange and attend a planning meeting to develop/update the Care Plan within 10 days following the child’s/young person’s admission to the home. The first 12 weeks of placement will usually be an assessment period. This is to ensure that the child or young person is placed appropriately to their needs and to develop the individual care plan. Further planning meetings or placement reviews will be held at a minimum of 12 weekly intervals, following the initial meeting, in order to review the placement and to ensure that we are offering a service that will provide the best possible outcomes for the child/young person. |
